Just found this over on the VRAT forum. Looks like it's a Baird 8721 almost certainly fitted with the 8500 chassis.

Bumped ; it is a Thorn 19" colour set, we had one from D.E.R. from 1976 until 1982 ; it took a video recorder on button4 from 1981 but when channel 4 started, it was changed for a new ferguson 26" with teletext.
On the rear panel , I remember, it had a contrast control and some sort of circuit breaker reset device. That never tripped in all the while we had it, nor did it go wrong in the 6 years, and it wasn't new when my Mum rented it. When the guy came to change it over, he remarked they it was the last one out on rental anywhere!
