I had a switched mode one of those adjustable voltage well warts with the same kind of PCB track switch.
It failed by going BANG.

Refugee,
I believe ypou had an extra additive to a normal wall wart.
Its known as audible annunciation of failure.

I translated the audio output from it.
It read "this wall wart is duff".
