Good practical examples of valve based distributed amplifier designs are used in the vertical amplifier stages of the Tektronix 545A and the 548A Oscilloscopes of the late 1950/ early 1960's. As far as I remember there is a good description of these in the Tektronix instruction/service manuals which can be downloaded from the BAMA website.
