I know of one example. My father wrote occasional pieces in the 1960s and '70s for Practical Wireless and Practical Electronics, and one or two other mags. He used his own name most of the time. But when that would have meant him getting published in consecutive issues (or even, I think on one occasion, the same issue) he used my first name and my sister's middle name to create a new name.
