Almost certainly a duff stylus I'd say.

The tips can get chipped, or completely knocked off sometimes, just leaving the stump to which they were bonded protruding from the metal cantilever.

I don't think the horizontal offset of one tip compared with the other has anything to do with it.

PS This thread was really about identification of BSR decks, so if you could find out the model number of yours for us, that would be great.
